Proxy program hosted from home computer?

I used to have a program on my computer that allowed me to access blocked sites at school...It automatically installed all the filesand put a file on my usb key with the link to my private proxy...does anyone know where to get this i lost it when i got new computer and i forgot the namei remember that i just had to click on my ip address with a port attatched and it took me to a plain looking proxy pagePLEASE ANSWER AS SOON AS POSSIBLE I NEED THIS FOR TOMORROW
